Register for a LIOS workshop

When you are ready to register for a LIOS workshop, download the form and submit by fax or mail. All registrations must be submitted in writing.

Payment/Cancellation Policy (as of 4/1/08)
All registrants must submit a Registration Form with Payment prior to first day of seminar. Phone registrations are not accepted. All registrants may request one (1) transfer only. Cancellations received prior to the two-week deadline will receive a full refund. Those who cancel within 14 days of workshop are not eligible to receive a refund, but may transfer registration to the next scheduled workshop (on a space-available basis). Cancellations without a transfer request will be charged a $50 fee to cover materials and other related expenses. All requests for transfer must also be submitted in writing to liosadmissions@lios.org. If attendance at the next scheduled workshop is not possible, no further accommodations can be made. All workshop “no shows” forfeit their full registration cost. No transfers are allowed. Walk-in workshop registrations cannot be accommodated. To indicate that you have read, understood and accepted these policies, please sign and date the registration form. Your registration is not complete without your signature.
